To many, this term is unfamiliar. Post-Acute Care is intense nursing and rehabilitation for individuals who have been hospitalized and require further nursing care and rehabilitation services.  This care is provided in a relaxed and comfortable setting with the intent of returning home.

Post-acute care is comprehensive inpatient care designed for an individual who has had a recent illness, injury, or exacerbation of a disease process. It is goal oriented treatment rendered immediately after, or instead of, hospital-based care.

An interdisciplinary approach is used to formulate a goal-oriented plan of care for each patient so that they may achieve their highest level of independence as quickly as possible.
