Services are provided by Registered Nurses, Licensed Practical Nurses and trained nursing technicians consistent with the personalized treatment plan established with your physician. Nursing interventions are developed based on a nursing assessment and are specific to each client. We work closely with your physician to achieve the best outcomes possible and provide a premier level of home care services to you, including;
  • Assessing blood pressure and other vital signs
  • Changing catheters
  • Medication teaching and management
  • Post-surgical care
  • IV medication administration
  • Pain Management
  • Wound Care
  • Teaching self care in chronic disease
A licensed member of our team is available 24 hours a day for urgent situations and to answer any questions you may have. Our extensive capabilities of skilled services and staff of knowledgeable professionals allow our patients to recover in the comfort of their own homes.

Our licensed Physical Therapists help regain physical function in order to preserve your ability to move about safely and independently. The spectrum of services a therapist provides is inclusive of balance training, gait and transfer training, strengthening, flexibility, functional mobility skills, use of mobility equipment and pain control. The Physical Therapist will make home modification suggestions if needed.

Occupational therapy services focus on the individual needs of the patient, and focus primarily on training for activities of daily living. Restoration of daily living skills is accomplished by our licensed Occupational Therapists. Through teaching of innovative methods to perform these skills and use of assistive devices, their goals are to promote safety and independence.

Regaining abilities such as dressing, grooming, eating, energy conservation, therapeutic exercise to re-building strength, and cognitive abilities are a main focus. Based on the needs of the patient, our Occupational Therapist may also make home modification suggestions.

Our licensed Speech Therapists provide an effective program to assist in the recovery of communication skills. They also evaluate and treat swallowing problems that are medically related

Our Certified Nursing Assistants support patients with activities of daily living and personal care including bathing, dressing, grooming, ambulation, and transfers for a short period of time until the patient regains independence or the family is able to continue with assistance.

Our Medical Social Workers have extensive knowledge of community resources to assist persons in need. They assist in solving problems that arise due to illness, the adjustment to illness, social, financial and transportation issues.

Our homemaking services are designed to help maintain normal household functioning and may include:
